Steps for Creating the Plan

1. Identify an Emergency contact person who can contact your local person who knows the animal plan

2. Identify a local contact person who has a copy of the plan and knows how funds are accessed

3.Identify a caregiver or local rescue

4. Fill out the form with complete information about your pets

5. Give a copy of your form to your emergency contact, local contact and caregiver, if identified

6.Put a paper copy of your form with your end of life documents
